	Username (This is the name you use to log into the employee portal and webmail. Ex. fsa\myusername)

	Employee Id (Your FSA employee ID.)

	Last 4 digits of your Social Security Number.

	Enter your new password. (See Password Requirements)

	Re-enter your new password. (Must match what you type in the box above.)
